Thank you for deciding to fill out the member survey.  If you have not already done so, please Create an Account.  If you have already been using our current site (on Ning.com), please use the same email address as you used on that site.  Doing so may help us to link this site to the original site later on.

After you have created an account you will see a link to the Survey, which will appear in the Main Menu, on the left side of this page.  Please click that link, which will take you to the survey form.
